Description (Service) |
Provides primary health care, including social, addictions and mental health services, chronic disease management, health promotion and disease prevention to rostered patients in the town of Hanover and surrounding area. Team includes nurse practitioners, registered nurses, addictions/mental health counsellor, registered dietitian, psychologist, pharmacist, occupational therapist and social workers.
Services and programs offered include: - prenatal classes
- breastfeeding and infant feeding support
- medication review
- support to quit smoking or vaping
- lung health clinic
- nutrition counselling
- range of primary care health assessments and treatment supports for all ages and ranges of urgent or chronic health concerns
- preventative care screening (breast, colon, cervical)
- sexual/reproductive health care
- mental health counselling
- care coordination and/or information and referral to other agencies
- cardiac rehab teaching and activation
- three year old well child clinic to screen for developmental milestones
- memory clinic to assist patients and caregivers with assessing memory concerns and connecting with community supports
- mobility assessment (risk of falling)
- exercise groups (Boosting Balance and Stride Alive)
- palliative care support
- flu shot/COVID-19 vaccine clinics in October/November
Individual counselling, as well as individual health assessments and treatments, are available by appointment and by referral for rostered patients of Hanover physicians.
Also offers health promotion, wellness and disease prevention educational group programs for anyone in the community, no referral required.
