| Hours | Home Care Office: Mon-Fri 8:30 am-4:30 pm, services available at other times, if required. Clinic: Mon-Fri 9 am-4 pm |

| Eligibility | Anyone requiring clinic physiotherapy services. OHIP coverage for those 19 years and under, 65 years and older, ODSP or Ontario Works recipients and people hospitalized for at least one night due to an injury. |
| Application | For services through Ontario Health atHome referral: Call Ontario Health atHome South West Intake. For private services: Call Closing The Gap office. No referral required for physiotherapy sessions. |
