Description (Service)

Provides a variety of community-based services for seniors and adults living with disabilities or chronic illnesses. Helps clients and their caregivers maintain independence, enhance their quality of life and keep them living in their homes longer.

Services include:

  • Transportation - for those who have difficulty accessing existing transportation options
  • Adult Day Away - socialization, recreation, medically supervised and caregiver respite
  • Overnight Respite - multi-day, medically supervised, away-from-home caregiver respite
  • Housekeeping/In Home Respite - support for those needing assistance with household chores and caregiver relief during daytime hours
  • Hot and Frozen Meals on Wheels - locally prepared nutritious and delicious meals
  • Friendly Visiting - scheduled social visits with a volunteer who has common interests
  • LEGHO - Let's Go Home - Transitional support post hospital discharge
