Provides primary health care services for patient care and offers health promotion and disease prevention. Registered Nurse, Social Worker, Occupational Therapist, Dietitian, Kinesiologist, Midwife and Nurse Practitioner offers:

  • Hypertension program assisting patients with blood pressure measurement, self care and management education
  • Dietitian Services
  • Occupational Therapy
  • Fall Prevention
  • Exercise Programs (Boosting Balance, Stride Alive)
  • Same-Day Appointment Clinic operated by Nurse Practitioner
  • Mental health counselling
  • Primary Care Midwifery
  • Lactation and Feeding Support
  • Memory Clinic (physician/NP referral)
  • Cardiac Rehabilitation (physician/NP referral)
  • Group Health Promotion Education Programs

Hours Mon-Fri 8:30 am-4:30 pm
Specific Programs and Services: hours vary
Areas Served Huron-Kinloss ; Kincardine
Eligibility Programs are for residents of Huron-Kinloss and Kincardine and self-referral is accepted for most services.
Cardiac Rehab requires a Physician or Nurse Practitioner referral.
Memory Clinic only accepts referrals from Kincardine Physicians and Nurse Practitioners.
