| Description (Service) | Helps individuals to identify meaningful goals and effective ways for each person to reach those goals. Team members work alongside individuals, enabling individuals to exercise their rights and to enjoy a meaningful life in their community while striving to achieve their full potential. Team members work to promote health, housing, purpose and community. Uses a holistic, strengths-based approach to support individuals towards living a full, independent and satisfying life. This is a time limited services that is focused on supporting individuals to live as independently as possible. |

| Hours | Office: Mon-Fri 8:30 am-4:30 pm, some evening services by appointment, subject to program resources. |

| Eligibility | For individuals living with a serious mental illness and/or substance use disorder, and have a willingness and a commitment to work on personal recovery goals.
