Description
Description (Service) | A program for individuals with serious mental health challenges who are looking for coordination, advocacy, and life skill development support. Assists individuals in getting connected to new and appropriate resources in their communities and develop skills that will assist them in living a fulfilling and meaningful life. Case Managers meet with individuals in their home or community and help them identify and work towards personal recovery goals that will improve their quality of life. |

Eligibility | 16 years and older. |
Application | Referrals accepted from hospital, inpatient units, family doctor, community organizations and health services. Call the Central Referral and Intake Team. |
