Description
Description (Service) Program offers support to young adults as they transition out of the child welfare system into independent living 
Using a client centered approach a Youth in Transition worker will assist young people to identify and develop a plan based on their personal goals 
 
This plan could include: 
* securing local affordable housing 
* finding education and employment resources to help cover the cost of post secondary education and training, or to find a job 
* identifying skills training, such as financial literacy courses and meal planning 
* accessing health and mental health services such as being connected with a family doctor and counselling 
* locating legal services, including advice for youth in the justice system
